Cost Savings

What is High Intensity Focused Ultrasound or Focal HIFU?

HIFU is a procedure where the temperature inside the prostate is raised to 85 degrees Celsius using a focused ultrasound beam.

After spinal anesthesia with intravenous sedation the physician places a probe into the rectum. This probe emits a beam of high intensity focused ultrasound. At the point where the ultrasound is focused (focal point) the sudden and intense absorption of the ultrasound beam quickly raises the temperature which destroys targeted cells.

The area destroyed by each beam is very small and precise. By repeating the process and moving the focal point it is possible to destroy the prostate tissue. The treatment takes from 2 to 2 1/2 hours depending upon the size of the prostate and is performed on an outpatient basis.

Clinical Results

An independent European study of 402 patients with localized prostate cancer that were treated with HIFU resulted in 87.2% of patients having no cancer found in their biopsies and normal PSA levels in 81.4% of treated patients.

Another study showed that 86% of HIFU treated patients had no cancer found in their biopsy tests 9 years after treatment.

Is the HIFU procedure painful?

The treatment itself is not painful as it is carried out under local anesthetic (epidural) or sometimes general anesthetic.
Pain at the end of the treatment is rare, although most patients feel a slight discomfort which disappears after a few days. The procedure is minimally invasive so there are no wounds and patients do not experience the burning sensation often associated with radiotherapy. Unlike radiation HIFU treatment can be repeated if necessary and will not prevent radiation treatment or a radical prostatectomy in the future because it does not damage the surrounding tissue, like radiation.

What happens after treatment?

Most patients can go back to eating normal food the evening after treatment and are discharged from hospital the next day. The urinary catheter is generally removed three or four days later. Medication may be prescribed for the following eight weeks to prevent any infection of the urethra or bladder.
In the period after treatment you may experience some discomfort including mild bleeding at the start of urination, frequent and sometimes urgent urination, urine leakage during physical exertion or coughing and sometimes the elimination of dead cancer cells in the urine. Infections with fever are rare but possible and require antibiotics.
These side effects disappear in the weeks following the treatment.

What long-term follow up is required?

Usually PSA levels are checked every three months and a biopsy is undertaken six months after treatment. If cancer cells remain (10 to 15% of cases) a second High Intensity Focused Ultrasound treatment is likely to be given six months after the first treatment.

What are the side effects?

Trials of High Intensity Focused Ultrasound treatment have found few side effects. The healthy surrounding cells are not destroyed along with the cancerous cells and the procedure is minimally-invasive (does not involve an incision through the skin). This means only a short hospital stay is required and you will be able to return to normal activity after only a few days. High Intensity Focused Ultrasound patients do not experience a burning sensation associated with radiotherapy.

High Intensity Focused Ultrasound treatment after radiotherapy

High Intensity Focused Ultrasound treatment can be given to patients whose cancer has returned after radiotherapy, although some patients are not suitable (about 5% cases) due to abnormal thickening of the rectal wall or because the cancer has spread.
Results for this type of treatment are consistent with other options such as cryotherapy or surgery (Radical Prostatectomy). In a study conducted at Edouard Herriot Hospital in France no cancer was found in biopsy tests for 73% of patients and the cancer was controlled in 50% of cases.
The risk of incontinence is higher and the recovery time longer compared to patients undergoing HIFU treatment as a first option.


HIFU treatment is currently not available in the US although it is in stage 3 clinical trials with the FDA. HIFU treatment has received regulatory clearance in Europe, Canada, Australia and else where.

Robotic Surgery

Robotic surgery or computer assisted surgeries are technologically assisted surgeries that aid in surgical procedures. The main objective in using a robitically assisted surgical system is to reduce or eliminate the tissue trauma traditionally associated with open surgery. The majority of surgeries can be improved through the use of robotic surgery assistance which utilize minimally invasive techniques. Some major advantages of robotic surgery are precision, miniaturization, smaller incisions, decreased blood loss, less pain, and quicker healing time. Further advantages are articulation beyond normal manipulation and three-dimensional magnification, resulting in improved ergonomics. Robotic techniques are also associated with reduced duration of hospital stays, blood loss, transfusions, and use of pain medication. For example In traditional open-heart surgery, the surgeon makes a ten to twelve-inch incision, then gains access to the heart by splitting the sternum (breast bone) and spreading open the rib cage. The patient is then placed on a heart lung machine and the heart is stopped for a period of time during the operation. This approach can be associated with postoperative infection and pain, and prolonged time to complete recovery. Because patient recovery after robot-assisted heart surgery is quicker, the hospital stay is shorter. Reduced recovery times are not only better for the patient, they also reduce the number of staff needed during surgery, nursing care required after surgery, and, therefore, the overall cost of hospital stays.

Because of the cost of a robotic surgery assistance devices such as the Da Vinci System, the additional disposable supply costs and the additional training required by the surgeon it is often difficult to purchase and implement the technology in North American hospitals. Subdural Hematoma Treatment (SDH) – Treatment of a subdural hematoma depends on its size and rate of growth. Some small subdural hematomas can be treated by inserting a temporary small catheter through a hole drilled in the skull and sucking out the hematoma. In other cases, surgical opening of the skull may be necessary.

Ventral Discectomy – This refers to the surgical removal of herniated disc material that presses on a nerve root or the spinal cord.

Their are many different versions of the Gastric bypass surgery, but in short, it is a procedure that divides the stomach into a small upper pouch and a lower pouch. Thus creating a reduction in the functional volume of food the stomach can hold, creating an altered physiological and physical response to food.

What is a lap band procedure?

CCSVI Liberation Therapy

Chronic Cerebro Spinal Venous Insufficiency (CCSVI) in Belgium

This procedure, which was developed by Dr. Paulo Zamboni is believed to play a role in the cause of Multiple Sclerosis. CCSVI or Liberation treatment is an angioplasty of the veins which return blood flow to the brain and subsequently the draining of the central nervous system. Dr. Zamboni’s research claims that in MS patients diagnosed with CCSVI, the azygos and IJV veins are stenotic in around 90% of cases. Dr. Zamboni theorized that malformed blood vessels cause increased deposition of iron in the brain, which in turn triggers autoimmunity and degeneration of the nerve’s myelin sheath.

In the case of CCSVI, treatment is not readily available or affordable in North America, leaving many searching for alternatives in second and third world countries. What happens if my my veinography is negative and I do not have blockages or CCSVI?

If no blockages are found, the client will be billed $200 for the initial doppler screening and diagnoses.

Why do you encourage clients to get an MRV?

Some patients may need an MRV to check the morphology of the stenosis for a detailed diagnosis and to be able to outline the best treatment plan. However, the MRV is also a useful tool as it is a “snap shot” of the vein blockages and can be used as a reference tool to monitor future vein developments.

Additional material costs could be billed to the client (for example stenting) if it is deemed to be needed during surgery. Stenting is only performed when it is deemed to be medically necessary (statistically less than 3% of the time).

Why Belgium?
